We collaborate with the client to identify crucial aspects for a smooth transition. This could include securing stable housing, identifying employment opportunities, and building vital support networks. We also explore potential resources and programs that address any underlying issues that the individual may have, such as substance use disorder or mental health concerns. Our goal is to equip the individual with the tools and support necessary to reintegrate seamlessly and build a positive future.
